New Patients and Your First Visit
We welcome you to our practice and believe you will be pleased with your choice of IMGI for all of your health care needs.
We have some forms and paper work which should be filled out prior to your appointment, and these forms can be downloaded from our website. We can also fax them to you, if you prefer. Please fill them out and bring them in with you. We also ask that you bring in a list of all prescription medications you use, along with their dosing. In like fashion, it is very helpful if you bring in all of the supplements you normally take, so we can review them with you. If you have other physicians who have been treating you, please bring in their names and addresses so that we can send for your records. If you can obtain them ahead of time and bring them with you, it would be very much appreciated.
Depending on the type of visit, the amount of time can vary from 30 to 90 minutes. We want you to have the use of the entire amount of time we have set aside for you, so we ask that you arrive 30 minutes prior to your actual appointment time. This allows us to process the necessary paperwork and get you in with your provider in time to begin the appointment at the designated time.
If you arrive late, we regret that your time with the provider will be shortened. We try very hard to stay on time for the convenience of all. If you have extra time for waiting, we welcome you to enjoy our purified water or tea served in our waiting room, or to relax and read one of our many magazines. Our goal is lower your stress, not elevate it.
Please bring in your insurance information, so that we may bill our services for you, when applicable. Since we are out-of-network providers for all carriers other than Standard Medicare and TriWest, we will expect payment for all services at the conclusion of the visit. For your convenience, we accept most major credit cards. Payment financing is also available.
