I truly believe that physicians must provide a leadership role in the promotion of healthy lifestyles. That said, I have decided that I must do more in my own life, so that I can be the best possible role-model for my patients. Sadly, a recent survey of doctors revealed that among overweight doctors, only 5% of the time was a patient&#8217;s weight problem brought up at the visit. Even normal weight doctors brought up weight only 30% of the time. Given those numbers, one can only imagine the percentage discussing healthy sleep issues, exercise, and environmental toxin exposures.</p>
<p>Well, at IMGI, we aim to be different, and it is my job to lead the attack on bad lifestyle habits. Over the past few years, I have dramatically revamped my diet. Once a daily consumer of Diet Cokes and pretzels, I can honestly say that I haven&#8217;t touched either for several years. Now my diet, which many would think is boring (but I really love), consists of lots of mainly organic veggies, fruit, nuts, seeds, wild and organic meats, fish, poultry, and a bit of dairy, beans, lentils, and brown rice. I eat very large volumes of foods with mainly low-calorie density.</p>
<p>But as much as I have bragging rights to a healthy diet, I have been remiss in getting enough exercise and sleep. I have promised myself to change this and have been exercising with our trainer, Bob Tygenhof, 6 days a week. I now do both aerobic and resistance training. Actually, it is becoming more fun, though I probably would always choose to watch a good movie over exercise, if health weren&#8217;t important to me &#8211; maybe this will change some day. The sleep department is proving more challenging as there just are not enough hours in the day for me to do all I feel must be done. Nevertheless, I absolutely intend to increase my sleep to a minimum of 7 hours a night, a goal I usually, but not always, have been meeting.</p>
<p>So here is my challenge to all of you. Decide where you want to begin, or how you wish to improve, your lifestyle behaviors to enhance your overall well-being. Email me what you decide to do &#8211; improve your eating habits, exercise more, get more sleep, or reduce your toxic load, and I will personally email you back with concrete suggestions on how you can actively begin your journey towards better health. I will let you know how I am doing, and I hope you will keep me up-to-date on your progress.</p>
<p>So many &#8220;experts&#8221; today are saying that people will never change &#8211; they will forever behave in self-destructive ways. Please help me to prove those naysayers wrong. I have faith in you to really change and live a clean, wonderful, long life. Together we can accomplish anything we can dream.</p>

<p>NSAIDs are a class of pain-relief medications that are readily available, both by prescription and over-the-counter. Though widely used, they have been associated with some severe and possibly life-threatening complications. In this email newsletter we will explain those risks, so that you will be able to make truly informed decisions about your drug choices.</p>
<p>We are also announcing the start of fitness evaluation and training as an integral part of our healthcare services. The sedentary lifestyle led by most Americans is now seen as the number one cause of preventable disease, displacing smoking for the first time as America&#8217;s most destructive lifestyle habit. It&#8217;s time we all got moving again, and we&#8217;re going to do our part to make sure that happens.</p>

<h3 style="text-align: left; color: #91b587;">Update on the Risks of NSAIDs</h3>
<p>NSAIDs (Non-Steroidal Anti-Inflammatory Drugs) comprise a very large family of pharmaceuticals, which include Ibuprofen (Advil, Motrin), Naproxen <img class="alignright size-thumbnail wp-image-1568" style="margin: 10px 0 0 10px;" title="aleve" src="http://integrativemgi.com/medical-group-irvine/wp-content/uploads/2012/03/aleve-150x150.jpg" alt="" width="150" height="150" />(Aleve), Celebrex (also known as a Cox-2 Inhibitor), Mobic, Voltaren, Indomethacin, and many others. Though this group of medications is viewed by many as being very safe, in some cases side effects from their use can be life-threatening. Complications associated with NSAID usage involve the GI tract, the kidneys, and the cardiovascular system.</p>
<p>According to the American College of Gastroenterology, persons using NSAIDs are at a significantly increased risk of gastrointestinal complications. These negative side effects include injury to the intestinal lining that can result in ulcers and/or gastrointestinal bleeding. With millions taking NSAID pain medications every day, it is estimated that more than 100,000 Americans are hospitalized each year and 15,000 &#8211; 20,000 Americans die annually from ulcers and gastrointestinal bleeding linked to NSAID use.</p>
<p>A new study reported in Paris at the 27th Annual Congress of the European Association of Urology found that the use of NSAIDs is associated with an overall increased risk of prostate cancer and with the development of aggressive forms of that disease. The study could not state whether this finding was specifically due to NSAIDs or due to the varied reasons for their use, but it does merit further thought.</p>
<p>Last September, a study appearing in the Canadian Medical Association Journal reported that the use of NSAIDs in early pregnancy was linked to twice the risk of miscarriage. Subsequently, it is no longer recommended that pregnant women use NSAIDs at any time.</p>
<p>On November 2, 2011 a study presented at the American College of Gastroenterology Annual Scientific Meeting found that high doses of NSAIDs (not aspirin), longer duration of use, or greater frequency of use were all associated with an increased risk for Crohn&#8217;s disease and ulcerative colitis. Bruce Sands, MD, MS, chief of the Division of Gastroenterology and professor of medicine at Mount Sinai Hospital in New York, said he found the study </p>
<blockquote  style="font-size:1.3em"><p>a very provocative, interesting, and well-done work.</p></blockquote>
<p>&quot;The risk of NSAIDS for IBD [inflammatory bowel disease] has long been speculated due to their effects in decreasing barrier function of the gut epithelium. This is the first study to prospectively assess that risk in advance of onset of disease to my knowledge,&quot; he stated.</p>
<p>In September of 2011, a large systematic review from the United Kingdom and Canada confirmed that rofecoxib (Vioxx) is associated with a significantly increased risk of cardiovascular events, while also suggesting that diclofenac, (Voltaren) a widely used NSAID, poses an equivalent risk to patients. Although rofecoxib is no longer on the market, Dr. Henry, the senior investigator of the study, noted that diclofenac is still available, even though the risk profile of the two drugs is nearly identical.</p>
<p>Regulatory agents seem reluctant to remove the drug from the market, he said, adding that clinicians should take regulatory action on their own and not use diclofenac in certain patients. Similar risks may also be associated with the use of indomethacin and meloxicam.</p>
<p>&quot;We think this is a significant issue and that people should know about this,&quot; said Dr. Henry. &quot;It probably doesn&#8217;t matter in people at very low risk of a heart attack. But if you have had a heart attack or have a lot of cardiovascular risk factors, you probably want to avoid diclofenac because it increases the risk by about 40%. For somebody that has a 5% to 10% risk of having a heart attack, that&#8217;s a significant increase in absolute risk, particularly if there are other drugs that don&#8217;t seem to have that risk.&quot;</p>
<p>Another study addressed the use of non-steroidal anti-inflammatory drugs among patients with chronic kidney disease (CKD). This use occurs despite concerns about the nephro-toxicity of these drugs, according to a study published in the September/October issue of the Annals of Family Medicine.</p>
<p>&quot;Our findings suggest there may be large numbers of individuals with CKD, many of whom unaware of their disease, who may be at risk for further kidney injury through use of NSAIDs,&quot; write Laura Plantinga, ScM, from the University of California, San Francisco, and other members of the Centers for Disease Control and Prevention CKD Surveillance Team.</p>
<p>&quot;CKD screening should occur in those who use NSAIDs daily, and effective communication must be established concerning the risks of NSAID use among those with CKD. This education is warranted to prevent further kidney damage and progression of disease,&quot; according to the investigators.</p>
<blockquote style="font-size:1.3em"><p>If I go off of my NSAID, what can I do about pain?</p></blockquote>
<p>This is an excellent question, and the answer really depends upon each person&#8217;s unique situation. There are several extremely effective herbal remedies, all of which have been documented in excellent studies to be as efficacious as Advil, without the adverse concerns.</p>
<p>In addition to herbals, certain vitamins and minerals can help with pain modulation, and other approaches can also be considered, including acupuncture, meditation, breath-work, massage, and homeopathy. The bottom line is this: do not just take NSAIDs without truly understanding your individual risks, and consciously consider your alternatives the next time you think about popping an Advil.</p>

<blockquote style="font-size:1.3em"><p>What&#8217;s Fitness Got to Do with Medicine and Healthcare?</p></blockquote>
<p>Well, the obvious answer to that question is &#8220;everything.&#8221; As we&#8217;ve been saying for quite some time now, the most effective treatment for a whole host of lifestyle-related diseases is exercise. More effective than statins for lowering cholesterol, better than any of a slew of drugs for reducing high blood pressure, the surest way to reduce your risks of breast and colon cancer, and the most effective way to lower blood glucose levels and reduce your likelihood of developing diabetes.</p>
<p>AND THERE ARE VIRTUALLY NO BAD SIDE EFFECTS!!! Every other drug you can name comes with a packaging insert that reels off a long laundry list of possible adverse consequences. Exercise has two known side effects. First, if you don&#8217;t know what you&#8217;re doing, you can hurt yourself. That&#8217;s why we recommend that you start off on the right foot with Bob. <h2><strong>FDA Warning Alert on Proton Pump Inhibitors</strong></h2>
<div>The FDA issued a warning alert yesterday, Wednesday, February 8th, that the risk of serious intestinal infections is increased in persons using the class of <img src="http://ih.constantcontact.com/fs052/1101855940444/img/80.png" alt="Nexium" name="ACCOUNT.IMAGE.80" width="200" height="71" align="right" border="0" hspace="5" vspace="5" />drugs known as proton pump inhibitors. These include Nexium, Prevacid, Prilosec (Omeprazole), Aciphex, Protonix, and Dexilant. The increased risk involves the development of diarrhea due to <em>Clostridium difficile</em>. This is particularly worrisome, since new strains of this bacteria are both more virulent and anti-biotic resistant than were previous ones. The bacteria is also more widely dispersed than before, so exposure is far more likely. <em>C. difficile</em>infection is a potentially life-threatening event.The warning comes after a review of data from the agency&#8217;s Adverse Event Reporting System (see below). Medical literature has suggested such a link for quite some time, as indicated by <a href="http://www.medpagetoday.com/MeetingCoverage/ACG/22896">a meta-analysis</a> reported at the 2010 meeting of the American College of Gastroenterology.<img src="http://ih.constantcontact.com/fs052/1101855940444/img/81.jpg" alt="Prevacid" name="ACCOUNT.IMAGE.81" width="138" height="100" align="left" border="0" hspace="5" vspace="5" style="margin-right: 20px;" />PPIs block nearly all production of stomach acid. Although stomach acid has been vilified by the pharmaceutical companies, it is actually essential for the proper digestion and absorption of food nutrients. In addition, it protects the body from infections caused by pathogenic bacteria carried on the food we eat.At IMGI, we have been talking about the dangers of PPIs for several years and have successfully weaned many of our patients off these drugs.</div>
<h2>FDA Warnings</h2>
<div>Less than a year ago, the FDA issued several other warnings concerning this class of pharmaceuticals and declared that fully 70% of patients placed on these medications were done so inappropriately.<img src="http://ih.constantcontact.com/fs052/1101855940444/img/82.png" alt="Prilosec logo" name="ACCOUNT.IMAGE.82" width="153" height="76" align="right" border="0" hspace="5" vspace="5" />PPIs are associated with osteoporosis, Vitamin B12 deficiency, severe total body magnesium deficiency, and other nutrient deficiencies. Additionally, they are associated with higher levels of pneumonia and the development of gastrointestinal dysfunctions. Barrett&#8217;s Esophagus, considered a significant risk factor for esophageal cancer, is actually not reduced by these drugs, as the elimination of stomach acid does nothing to decrease the reflux problem and the fluids, minus the acid, which continue to reflux can be damaging to the esophagus.</div>

<h2>What should you do?</h2>
<div></div>
<div>One should never, ever just stop taking these drugs. Because of the severe reduction in stomach acid, the body compensates by putting out great quantities of the hormone gastrin in an attempt to stimulate the stomach into making more of the necessary acid. If the drug is suddenly stopped, the high quantities of this hormone will stimulate huge amounts of acid to be produced, resulting in great discomfort. Additionally, there are a small number of people who should not be taken off these medications or have never had their symptoms properly diagnosed.</div>

<p>After experimenting with several different newsletter formats over the past few months, we have settled on a &#8220;something old, something new&#8221; approach. We are returning to our previous graphic format, while adopting a single-topic focus for each issue.</p>
<p>This will allow us to be more concise, probe a little deeper, and provide you with usable information each and every month. Upcoming issues of this newsletter will zero in on topics such as headaches, sexuality, musculoskeletal problems, thyroid and other endocrine issues, cardiovascular health, menopause and hormonal issues, infertility, mood issues, menstrual problems, anti-aging issues, cognition, and so on.</p>
<p>New research articles will be highlighted in the newsletters, showing you not only what is known at the moment, but also giving you a glimpse of what the future may hold.</p>
<p>The focus of this month&#8217;s newsletter is on new research involving the effects of certain forms of pollution on our cardiovascular and metabolic health. This is extremely important, because cardiovascular issues account for more deaths in both men and women than any other cause, and we are in the midst of an obesity and type 2 diabetes epidemic of extraordinary proportions.</p>
<p>Interestingly enough, young people today don&#8217;t realize that in the past most people were of normal weight. I recently looked at my high school yearbook from 1970 and was more than a little surprised to find that there wasn&#8217;t a single obese student in my class of nearly 500. Only a few carried five or so unwanted pounds.</p>
<p>The purpose of this newsletter is to give you practical steps to take to avoid future exposures to these pollutants and to explain what your options are in dealing with past exposures. The urgency of this advice will, of course, be related to the severity of your previous contact with these chemicals.</p>
<p>We hope that this approach is of value to you, and we encourage you to share this knowledge with your friends and loved ones. We are all on the same very interesting and very challenging journey through life. Let&#8217;s make it the best and longest ride ever!!!</p>

<h1><span style="color: #99cc00;">The World Wide Epidemic of Type 2 Diabetes (T2DM) and the Fishy Business of Persistent Organic Pollutants</span></h1>
<p>The incidence of T2DM throughout the world has been rising at an alarming rate. Generally this is believed to be due to unhealthy lifestyles and, consequently, there has been an emphasis on increasing exercise, changing dietary habits, and promoting weight loss. This, of course, is sound advice for a wide range of health and medical reasons.</p>
<p>But critical information from recent studies has added to our knowledge on this complex subject. These studies reveal that the presence of several persistent organic pollutants (POPs) can confer an even greater risk for developing type 2 diabetes than some of these established lifestyle risk factors.</p>
<p>In fact, evidence now suggests that obesity might only be a significant risk factor when adipose tissue contains high amounts of POPs. Pollutants such as chlorinated pesticides, and polychlorinated biphenyls, in particular, are strongly associated with the development of metabolic syndrome, insulin resistance, and T2DM. Various seafood, and <img class="size-full wp-image-1576 alignright" style="margin-top: 10px;" title="Dietary Sources of POP" src="http://integrativemgi.com/sandbox/medical-group-irvine/wp-content/uploads/2012/02/fish.jpg" alt="Fish" width="150" height="100" />particularly farmed Atlantic salmon, have been shown to be significant and common dietary sources of POPs, with resulting blood sugar dysregulation and metabolic dysfunction.</p>
<h4>Persistent Organic Pollutants (POPs):</h4>
<p>These are organic compounds which are resistant to environmental degradation. In fact, data shows that the POPs found in farmed salmon would take between 50 and 75 years to clear from our bodies. Unfortunately, these pollutants persist within the environment and bioaccumulate in human and animal tissue and biomagnify within the food chain. A United Nations council originally created a list of 12 POPs known as the &#8220;dirty dozen&#8221; which has since grown to include additional POPs. Some of the names from this long list with which you may be familiar with are DDT, Polychlorinated Biphenyls (PCBs), Chlordane, and Hexachlorobenzene.</p>
<h4>A few of the recent studies are referenced below.</h4>
<p><strong>Link between dietary POPs and obesity</strong></p>
<ol>
<ol>
<li>A study published in the journal Science looked at the effects of feeding rats one of four diets (each with the same calorie content) for 28 days &#8211; either regular rat chow or one of 3 high-fat diets, with added corn oil, crude Atlantic salmon oil, or Atlantic salmon oil which was highly purified to remove all POPs. The rats fed the crude Atlantic salmon oil accumulated visceral adipose tissue and developed severe hepatosteatosis (fatty liver). They also developed insulin resistance. On the other hand, rats fed the purified salmon oil lost weight and had reduced amounts of fat in the liver. The study demonstrated a causal link between dietary POPs and visceral obesity, fatty liver, and insulin resistance.</li>

</ol>
<p><strong>Better insulin sensitivity</strong></p>
<ol>
<ol>
<li>An 8-week study involving mice published in the journal Environmental Health Perspectives showed Lab mouse similar findings. The mice were fed one of several diets: control diet, high-fat diet, with farmed Atlantic salmon, Western diet, Western diet with farmed Atlantic salmon, and a high-fat diet with farmed Atlantic salmon which had been processed to remove most of the POPs. The mice were monitored for weight and glycemic control. <img class="alignleft size-thumbnail wp-image-1580" style="margin-top: 10px;" title="rat" src="http://integrativemgi.com/sandbox/medical-group-irvine/wp-content/uploads/2012/02/rat1-e1328641038501-150x150.jpg" alt="" width="150" height="150" />Mice fed the contaminated salmon in either the high-fat or Western diet had increased insulin resistance, visceral fat accumulation, glucose intolerance, and a reduction in muscle glucose uptake in response to insulin. The mice fed the exact same diet, but with the reduced POP salmon, showed much better insulin sensitivity and glucose tolerance.</li>
</ol>
</ol>
<p><strong>Study of Native Americans</strong></p>
<ol>
<li>Another study published in Environmental Health Perspectives looked at the levels of PCBs and other contaminants in the blood of Native American Mohawks who ate a lot of fish from the highly polluted St. Lawrence River. The rate of T2DM in those studied was over 3 times the rate of the general U.S. adult population and there were significant associations found between T2DM and the high levels of PCBs found.</li>
</ol>
<h2>Take-Away-Message:</h2>
<p><img class="alignleft size-full wp-image-1575" style="margin-top: 10px;" title="salmon" src="http://integrativemgi.com/sandbox/medical-group-irvine/wp-content/uploads/2012/02/salmon1.jpg" alt="Salmon" width="150" height="100" /></p>
<p>We recommend that you eat only wild salmon, rather than farmed Atlantic salmon. The wild variety has been studied and contains 1/10th the amount of POPs, along with a different and safer profile of pollutants. If you must eat farmed salmon, do so no more than once a month. Additionally, do not eat fish from polluted bodies of water such as the Great Lakes. Farmed trout and Tilapia are acceptable, and wild halibut and black cod are good choices.</p>
<p>Take substantial supplements of purified Omega 3 from fish oil, but know the source. Virtually all fish oils are now free of mercury, but this is not true for POPs. We can vouch for the products we carry, but be careful if you shop only for price when selecting a fish oil product. You may be taking a highly concentrated dose POPs!</p>
<p>In the near future we will be offering laboratory testing for environmental pollutants. Although these tests, run by Metametrix Clinical Laboratory, are not covered by insurance (what a surprise!) you may find them worthwhile. Sadly, becoming aware of your own personal levels of environmental pollution should now become a part of your medical evaluation.</p>
<p>We believe that all women anticipating pregnancy in the future should acquire this information. How these POPs affect the developing fetus is a separate topic with enormous implications for the future of human-kind. Stay tuned for more on that and other related topics.</p>

<p>I certainly hope that you all had a very enjoyable holiday season and that you are now ready to tackle 2012 with optimism, resolve, and a strong dedication to making this new year the happiest and healthiest possible.</p>
<p>In order to help you in these endeavors, we&#8217;ve been working on a number of new programs and improving some of our existing ones. I can share with you that those being launched early in the year will include exercise and fitness programs, detoxification programs, plaque reversal cardiovascular programs, and updates to our popular weight loss programs.</p>
<p>As everything in the body is interconnected, there is overlap between the programs. What is good for your musculoskeletal system is good for your digestive system is good for your cardiovascular system is good for . . . well, you get the idea. <h4>Detoxification &#8211; Cleansing the Body From the Inside Out</h4>
<p>Detoxification, more accurately known as biotransformation, is the body&#8217;s natural, continuous process of eliminating and neutralizing waste products. Flaws in this essential process have now been linked to many neuro-degenerative diseases such as Parkinson&#8217;s disease, along with increased risks of cardiovascular disease, autoimmune disease, and cancer.</p>
<p>Under the best of circumstances, detoxification is a demanding task, since the body naturally produces many highly reactive and toxic substances &#8211; our own metabolic waste material. <h4>12 Great Ways to Get the Body You&#8217;ve Always Wanted</h4>
<p>Here are a dozen suggestions to help you lose fat, feel better, and look the way you should:</p>
<ol>
<li><strong>Slow down your eating.</strong> Set a timer, if necessary, to help pace your eating. Make each meal last at least 20 minutes, preferably 30 minutes or more. You will enjoy your food more, and your brain will have the time it needs to tell you when you have eaten enough and should stop.</li>
<li><strong>Sleep more.</strong> If you get less than 7 hours of sleep at night, add an hour in bed to discourage night eating and to allow your body to heal and de-stress. Lack of sleep stimulates the adrenal gland to produce more cortisol, which, in turn, increases appetite and slows your metabolic rate.</li>
<li><strong>Substitute vegetables for starches at all, or most, meals.</strong> Use a wide variety of vegetables &#8211; try to include 3 different ones with dinner.</li>
<li>Start dinner with a big salad topped with a dressing of yummy vinegar or lemon juice, along with a touch of organic olive oil. On occasion, replace the salad with a vegetable soup made with broth.</li>
<li><strong>Just say no to sugar, manufactured food, chemical food, and food with high fructose corn syrup.</strong> They never help your body and always hurt it. Honestly, it&#8217;s time to say good-bye to these faithless friends.</li>
<li><strong>Drink green tea often.</strong> It has wonderful phytonutrients and anti-oxidants, and has been shown to trigger fat burning.</li>
<li><strong>Eat at home.</strong> Studies show you&#8217;ll eat half the calories and consume a small fraction of the sodium you otherwise get when eating out. Get friendly with your food and explore new places to shop, such as farmers markets.</li>
<li><strong>When you must eat out, don&#8217;t be shy.</strong> Order your food to be prepared in a healthy manner, usually baked or broiled with any sauce on-the-side. Substitute vegetables or salad for the starchy carb dish.</li>
<li><strong>Learn some form of meditation, breathwork, self-hypnosis, or guided imagery.</strong> Lowering stress (which lowers the adrenal hormone Cortisol) has a major impact on your ability to lose weight and keep it off.</li>
<li><strong>Measure your food portions until the sizes become automatic.</strong> &#8220;Supersizing&#8221; of portions has become the norm, a leading cause of the obesity epidemic.</li>

<h4>American Diabetes Month</h4>
<p>November is American Diabetes Month, and, with the holidays fast approaching, it is essential for all of us that we protect our bodies by establishing healthy lifestyle habits. In fact, an important study in a recent issue of the &#8220;Annals of Internal Medicine&#8221; suggests that a combination of five healthy lifestyle factors may help reduce the risk of type 2 diabetes, even when there is a positive family history and an overweight status. This is important new information because an estimated 25.8 million Americans &#8211; over 8% of the entire US population &#8211; now has diabetes.</p>
<p>Though previous research had linked several healthy lifestyle factors to a lowered risk for type 2 diabetes, these narrowly focused studies typically looked at the impact of just one risk factor at a time. This time, a research team for the National Heart, Lung and Blood Institute compiled data that considered the combined effects of five risk factors: healthful eating, maintaining an optimal body weight, engaging in recommended amounts of physical activity, not smoking, and keeping alcohol use to no more than one drink per day for women and two for men.</p>
<p>Data from more than 200,000 healthy adults between the ages of 50 and 71 was compiled. Each person completed extensive surveys for three years during the mid-1990s about their diets, lifestyles, medical conditions, and other issues. They were then followed for 11 years to see if they developed diabetes.</p>
<h4>Reduce diabetes risk by up to 84%</h4>
<p>The results suggest that if Americans were to adopt multiple healthy lifestyle factors, they could reduce their diabetes risk by up to 84%!</p>
<p>During the course of the study, 9.6% of men and 7.5% of women developed new-onset diabetes. Among men and women who were overweight, the odds of developing diabetes were 31% lower for men and 39% lower for women for each additional healthy lifestyle factor incorporated by the individual.</p>
<p>For overweight persons whose diet score, physical activity level, smoking status, and alcohol consumption were considered low-risk, the odds for getting diabetes were 39% lower for men and 57% lower for women. Maintaining a healthy weight led to the greatest protection &#8211; at a standard, healthy weight, men were 72% less likely to develop diabetes and women were 84% less likely to be at risk. Similar associations were observed among participants with and without a family history of diabetes.</p>
<h4>Our Take On This: </h4>
<p>This research provides an important incentive for people to adopt a more healthful lifestyle. The health care team at IMGI continues to work hard to develop cutting-edge programs to help each patient develop optimal health. <p>Approximately two weeks ago, we were all shocked by a story appearing in the national news media warning that women taking vitamins had a higher mortality than women not taking vitamins. Subsequent to this, many patients called our office, concerned that their use of vitamins and supplements might be dangerous. In response to these concerns, we are forwarding one of the many well-reasoned rebuttals that appeared after this story was published.</p>
<p>Our take on this matter is no different from the position we have always held. We believe that good lifestyle choices &#8211; based on sound principles of good nutrition, exercise, stress reduction, minimizing exposure to environmental toxins, adequate amounts of sleep, and an upbeat emotional attitude &#8211; are the mainstays of a long, healthy, and happy life.</p>
<p>Nevertheless, obtaining adequate vitamins and minerals on a daily basis is a challenge for many people. Fast foods, processed foods, and our continued exposure to high levels of environmental toxins combine to make high-quality vitamin/mineral supplements a good idea.</p>
<p>Of course, all vitamins and supplements should be of the highest quality &#8211; low quality, hard-pressed vitamin tablets filled with suspect binders and synthetic (not bio-identical) vitamins are not a good choice. Although the study could not appropriately draw any conclusions due to its poor design, the best conclusion one could draw from this flare-up of silly science and media headline mongering would be that it is best to avoid substandard quality in supplements.</p>

<p>A recent study, &#8220;Dietary Supplements and Mortality Rate in Older Women,&#8221; published in the Archives of Internal Medicine, 2011, Volume 171(18):1625-1633, has caused some concern among the public about the safety of dietary supplements. We have thoroughly reviewed this study and share the following analysis in order to help you better understand the study&#8217;s design and its findings, and to help allay any concerns you may have.</p>
<h3>The Study&#8217;s Design</h3>
<p>This study is an analysis of data gleaned from 38,772 postmenopausal Caucasian women enrolled in the then-ongoing Iowa Women&#8217;s Health Study. The data for the study is based on the responses obtained through a self-administered questionnaire initially distributed in 1986, with two follow-up questionnaires distributed over the next 18 years, in 1997 and 2004. The questionnaires inquired about lifestyle practices, food intake, dietary supplement use, weight, smoking status, hormone replacement therapy, and the presence of diabetes or heart disease.</p>
<p>Although study participants were asked about their intake of dietary supplements, the study did not report how much of any specific nutrient was consumed. Nor was information elicited from the women regarding the chemical form of the supplement (e.g., picolinate versus sulfate) or the quality of the supplements that were taken. Furthermore, although the women were asked whether they took a &#8220;multivitamin,&#8221; the study does not define this term; i.e., the mineral, vitamin, and botanical content of the study&#8217;s universal &#8220;multivitamin&#8221; cannot be determined.</p>
<p>Finally, no attempt was made to verify the accuracy of the answers provided in the questionnaires, nor were any of the participating women asked why they were taking supplements, and no attempt was made to determine the impact of taking-or not taking-supplements on any specific individual.</p>
<p>One possible flaw to consider: it is well known that when an individual is diagnosed with a serious disease, such as cardiovascular disease or cancer, beginning or increasing the use of dietary supplements occurs commonly. If the new or increased supplement use were reported on a questionnaire and then subsequent mortality resulted because of the underlying disease, the situation could very possibly exist such that the individual&#8217;s death, while properly attributed to the disease, would also be &#8220;associated&#8221; with the use of a dietary supplement. Such an erroneous scenario is a highly likely flaw in the study&#8217;s design. </p>
<h3>The Study&#8217;s Findings</h3>
<p><a href="http://integrativemgi.com/medical-group-irvine/wp-content/uploads/2011/11/pills3-150x102.jpg"><img src="http://integrativemgi.com/medical-group-irvine/wp-content/uploads/2011/11/pills3-150x102.jpg" alt="multivitamins" title="multivitamins" width="150" height="102" class="alignright size-full wp-image-1407" /></a>The results of the study&#8217;s analysis claim to show a slightly higher risk of all-cause mortality associated with the use of multivitamins, iron, and copper. In weighing the study&#8217;s findings, however, it must be emphasized that the Iowa Women&#8217;s Health Study is a retrospective study of already collected data. It is not a prospective, controlled intervention study, i.e., it is not a &#8220;clinical trial,&#8221; in which participants would be given a specific dietary supplement or a placebo and then followed closely over time to observe not only the specific outcomes but also the factors possibly contributing to those outcomes.</p>
<p>As can only be surmised retrospectively, individual circumstances change over time, and a significant number of the women participants likely either changed or began new dietary supplement regimens over the course of the 18 years they were studied. And since there was no direct contact with the participants outside of the mailed questionnaires, general information surrounding individual deaths had to be obtained from public records; it was not ascertained by direct medical investigation.</p>
<p>With regard to iron and copper, it has been known for decades that both metals can be potentially toxic, as exemplified by the multi-system disease states that can result from hemochromatosis and Wilson&#8217;s disease, respectively. For this reason, many postmenopausal women, like men, probably should not take an iron supplement in the absence of anemia or a documented deficiency. At the very least, iron and copper supplements should be taken concurrently with antioxidants and/or antioxidant-rich foods to prevent a potential increase in oxidative stress. Each of these circumstances points to the merits of dietary supplements being recommended and overseen by medical professionals.</p>
<p>With regard to multivitamins, there is simply insufficient data that can be gleaned from the study to make any serious conclusion about the impact of multivitamins on mortality. This is due to the fact that there are literally thousands of different combinations of vitamins, minerals, and botanicals that can be considered a &#8220;multivitamin,&#8221; as well as a whole host of considerations such as quality, potency, dosage, protocol, and indications for use, among others.</p>
<h3>Practitioner and Patient Concerns</h3>
<p>We believe there are serious flaws in the methodology, analysis, and findings on which this study is based. Retrospective surveys such as this-in which people are asked to recall years of dietary habits or supplement use-are notoriously inaccurate. The only conclusion that can realistically be drawn is that a slight statistical association was found based on a limited data set of questionable reliability-and a simple association does not reflect causation. The study&#8217;s authors do not disagree, stating the following in their commentary to the study: &#8220;It is not advisable to make a causal statement of excess risk based on these observational data&#8230;&#8221; We heartily agree with this advice.</p>
<p>When made by a quality manufacturer, when recommended by a knowledgeable health-care practitioner, and when taken for the appropriate indication, dietary supplements promote, enhance, support, and help maintain overall good health and well-being. The &#8220;results&#8221; of the recent study do not diminish this conclusion.</p>
